Treatment-Specific Consent

​

Classic / Hybrid / Volume Lash Extensions

Eyelash extensions are applied to natural lashes using semi-permanent adhesive. Known risks include:

• Mild irritation, watering, or redness of the eyes, particularly with sensitive eyes or pre-existing allergies to adhesive ingredients (cyanoacrylate).

• Risk of lash damage if extensions are picked, rubbed, or removed incorrectly at home.

• Not recommended during active eye infections (e.g. conjunctivitis) — treatment will be postponed.

• Not suitable for clients with a known allergy to lash adhesive.

Aftercare advice given and understood: Avoid water, steam, and oil-based products on the lashes for 24–48 hours. Avoid mascara, eyelash curlers, and rubbing the eye area. Brush lashes daily with a clean spoolie.

Korean Lash Lift (with or without tint)

A Korean lash lift uses a perming solution to curl natural lashes, with an optional tint. Known risks include:

• Chemical solution may cause stinging, redness, or irritation if it contacts the eye directly.

• Risk of allergic reaction to the lifting, fixing, or tinting solution

• Temporary over- or under-curling if lash health varies; results are not guaranteed to be uniform on damaged or very fine lashes.

• Not suitable during pregnancy, breastfeeding, or active eye conditions.

Aftercare advice given and understood: Avoid water, steam, makeup, and rubbing the eyes for 24 hours. Do not use eyelash curlers on lifted lashes.

LVL Lash Lift (with or without tint)

LVL (Lift, Volume, Length) lifts and tints natural lashes using a chemical solution applied to a silicone shield. Known risks include:

• Possible stinging, redness, or swelling, particularly around the eye area.

• Risk of allergic reaction to lifting or tinting solution.

• Lashes may appear less curled if naturally very straight or short; results vary by natural lash condition.

• Not suitable during pregnancy, breastfeeding, or active eye conditions.

Aftercare advice given and understood: Avoid water, steam, and makeup for 24 hours. Avoid rubbing or sleeping face-down where lashes are compressed.

Brow Wax & Shape

Hot or strip waxing removes unwanted hair to shape the brow. Known risks include:

• Temporary redness, sensitivity, or minor bumps in the waxed area.

• Risk of skin lifting or burns if skin is sunburnt, recently chemically peeled, or using retinoids/Accutane.

• Not recommended on broken, infected, or irritated skin.

• Possible asymmetry corrected during the appointment with verbal client input before finishing.

Aftercare advice given and understood: Avoid touching the area with unwashed hands, sun exposure, and active skincare (retinoids/AHAs) for 24–48 hours.

Brow Tint

Semi-permanent dye is applied to colour brow hairs. Known risks include:

• Risk of allergic reaction, including swelling, redness, or chemical burns

• Tint may stain skin temporarily, especially on dry or textured skin.

• Not suitable during pregnancy, breastfeeding, or for clients with a known PPD/dye allergy.

• Result depends on natural hair colour and porosity; colour intensity is not guaranteed to be identical each time.

Aftercare advice given and understood: Avoid water, steam, and oil-based cleansers on brows for 24 hours. Avoid touching or scrubbing the area.

 Brow Lamination

A perming solution restructures brow hairs into a lifted, uniform shape. Known risks include:

• Risk of allergic reaction to the perming or fixing solution.

• Possible dryness, brittleness, or breakage of brow hairs, particularly if previously chemically treated.

• Temporary redness or irritation of the brow skin.

• Not suitable during pregnancy, breastfeeding, or on broken/irritated skin.

Aftercare advice given and understood: Keep brows dry for 24 hours. Brush into shape daily and apply brow serum/oil to maintain hair condition.

Patch Test Waiver 

I understand that a patch test is recommended before treatments. I choose to proceed without a patch text and accept the risks, including allergic reaction and sensitivity.

Acknowledgement and Waiver Statement:

I am over 18 years of age and consent to the agreement and to treatment or have a parent with me that consents to this service. This agreement will remain in effect for this procedure and all future procedures conducted by my technician. read English and understand that this consent agreement is legal and binding. I have read and fully understand all information in this agreement. I release my technician from all liability associated with this procedure, which is performed with the utmost attention to safety and proper application using tools and products that the technician has been professionally trained to use. I understand the aftercare instructions and will do my part to maintain my eyelashes/brows.

Photography, Data & Social Media Consent

Before-and-after photographs may be taken to track treatment results and for portfolio purposes. Your personal data is processed in line with UK GDPR.

By signing below, I confirm that:

• The information I have provided above is accurate and complete to the best of my knowledge.

• I have had the opportunity to ask questions about my treatment(s), risks, and aftercare.

• I understand that results vary between individuals and are not guaranteed.

• I understand that if a contraindication applies (pregnancy/breastfeeding, active skin or eye condition), the affected treatment(s) will not be carried out today, and I will not be offered an alternative consent route to bypass this.

• I will inform my practitioner immediately of any discomfort, stinging, or reaction during treatment.

• I will contact my practitioner promptly if I notice any adverse reaction following treatment.
